FIELD TRIPS | GENERAL INFORMATION

For Faculty
- Review guidelines for student domestic travel during pandemic
- Review domestic field trip guidelines, FAQs, and faculty field trip procedures
- Submit approval form/checklist to unit administrator and appropriate associate dean (Darrin Thornton for undergrad, Mallika Bose for graduate) (Deadlines: Domestic—5 days)
- Keep copies of the Incident Form for students only on your person while attending the field trip: guru.psu.edu/formpublic/8-05IncidentFormFRM3.pdf
- Make necessary transportation and lodging arrangements
- Collect student forms as listed below

For Departmental Staff/Responsibilities
- Assist faculty in completing the appropriate approval form/checklist (Domestic)
- Assist faculty with transportation and lodging arrangements
- Create roster for faculty to take on field trip
- Create roster for on-campus contact person
- Verify/Confirm completion of required student forms
- Verify/Confirm emergency contact information is complete
On-Campus Contact Person Responsibilities
- Have roster of all faculty and students on the field trip, including all emergency contact information
- Have copy of approval form/checklist with trip itinerary
- Have cell phone number of faculty on field trip
For Students: Complete and Provide Forms As Required
- Authorization to Charge Course-Related Field Trip Fees to Student Account
- Emergency Contact and Health/Welfare Form
- Field Trip Code of Conduct (overnight)
- Field Trip Release Form (if Student has Faculty approval for alternate travel arrangements)
- Memorandum of Understanding (overnight)
